General Job Statement:

The Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. A CHW assists individuals and communities to adopt healthy behaviors by conducting outreach for the organization to implement programs in the community that promote, maintain, and improve individual and community health. The Community Health Worker (CHW) will be responsible for helping patients and their families/caregivers to:

· Understand and implement discharge instruction(s) and care plan(s) in the home.

· Identify social determinants of health needs.

· Navigate and access community services and other support resources.

· Understand provided health education.

General Duties include:

Responsibilities include but are not limited to:

· Attend trainings and participate in outreach activities in grant service areas.

· Enroll clients in the program that have been identified from the area hospitals and community partners.

· Develop plan of care with patients.

· Conduct regularly scheduled visits and phone contacts with patients.

· Make appropriate referrals to providers, community resources, and services.

· Identify and communicate with patients care team (medical/primary care providers, behavioral health staff, recovery coach, and pharmacist as needed).

· Maintain knowledge of available resources and providers in service area.

· Support and encourage patients to adopt healthy behaviors.

· Maintain and ensure accuracy on all records and documentation is completed in a timely manner per policy.

· Collect data and participate in data outcomes and management strategies.

· Participate in community coalitions and community meetings.

· Participate on CHW state and local workgroups, committees, etc. as assigned.

· Develop and maintain relationships with medical providers and health department partners.

· Help patients in utilizing resources, including scheduling appointments, arranging transportation, and assisting with completion of applications for programs in which they may be eligible.

· Other duties as assigned.
